Suspension Techniques Rear Sway bar, who's got one?

Anyone using one of these? If so, do you have the install guide and the parts list? I've emailed ST 3 times with no response. The one i have has been sitting in the garage for... i dunno 5 years and I think some parts "disappeared".

I think that's what I have. Installed many years ago so the instructions are long gone. I could snap some pics for you if that helps.
